On 1-2 September 2025 the Consortium Meeting of the PRO.CARE.FUL project (Pro.active CARE F.or Users L.ife), co-funded by the Interreg Central Europe Programme took place at the CEI headquarters. The project aims at addressing existing gaps in home care accessibility and timely delivery by upscaling existing social care services and targeting the potential for preventing cognitive and physical decline. To achieve this, partners developed a strategy for the digital transformation of home care, tested the deployment of a hybrid care approach supported by digital solutions in five pilot actions, and elaborated action plans for the investment implementation.
This meeting represented an opportunity to discuss the outcomes of the hybrid model care testing, consisting in the application for prevention of cognitive and physical decline and in-person support through workshops, events and consultations to promote healthy lifestyles and habits. The model was tested in five locations, namely – Lublin (Poland), Stuttgart (Germany), Split (Croatia), Treviso (Italy) and Log Dragomer (Slovenia), involving seniors, formal and informal caregivers as well as local and regional authorities.
The evaluation of the social impact of the PROCAREFUL model is currently being performed by the CEI, through a multidimensional, multisectoral and multi-professional approach. A strong multi-stakeholder engagement on impact evaluation findings will inform the scaling-up strategies, accounting for the different characteristics of the involved areas, thus linking evaluation and policies strategies in a consistent analysis framework.
Last but not least partners started planning the roll-out of the project results and their capitalisation to ensure PROCAREFUL model adoption beyond the project boundaries and its conclusion, foreseen on 01 March 2026.
