The Consortium Meeting of the PRO.CARE.FUL project (Pro.active CARE F.or Users L.ife), co-funded by the Interreg Central Europe Programme is taking place at the CEI Headquarters. The project aims at addressing existing gaps in home care accessibility and timely delivery by upscaling existing social care services and targeting the potential for preventing cognitive and physical decline. To achieve this, partners have developed a strategy for the digital transformation of home care, have tested the deployment of a hybrid care approach supported by digital solutions in five pilot actions, and have elaborated action plans for the investment implementation.
As the testing period has just ended, partners are meeting in Trieste to evaluate the hybrid model results, first evaluation steps and forthcoming plans for its scaling up and knowledge transfer to areas beyond the project.
The CEI - Executive Secretariat is responsible for the evaluation of the social impact of the PROCAREFUL model and the development of policy guidelines for policy-makers and health-care stakeholders. As a result, the CEI will deliver a policy strategy for the adoption of the PROCAREFUL hybrid care model in the CE programme area.
